Your registration for MDO is not a confirmation of space available in a class. A confirmation email will be sent out as soon as available space has been determined. You can expect a 2 - 3 week delay for a confirmation. If it is determined that there is not a space available for your child, your registration fee will be refunded.

To register your child for MDO please follow the steps below.

Your registration is not complete until the following items have been completed:
1. Registration form below has been submitted.
 
2. Your child's immunization record or exemption form has been submitted.
The immunization record can be submitted by the following methods:
Bring to the CCC Office during normal business hours.
Scan and send by email here.
Mail to 2250 S Yukon Parkway, Yukon, OK Atten: MDO.

3. Registration fee is paid.
Registration fee (see info below) can be paid by one of the following methods:
Venmo - use the link below.
Cash or check. Please bring cash or check to the CCC Office during normal business hours or
mail to 2250 S Yukon Parkway, Yukon, OK Atten: MDO.

Please fill out all sections on the registration form with an asterisk *
If you do not fill out each section with an * you will receive a submission error. 

The registration fee is $50 for 1 child or $75 for 2 or more children enrolled in the program. For all payments, please specify the following in the memo: MDO Registration Fee, Child's Name
